Prior to your first appointment, you will be sent an email from Hampton Physiotherapy. Please click the link and digitally complete the required forms using a desktop, tablet, or smart phone. This will help expedite your first appointment.
New and Returning Patients: Scheduling An Evaluation
Please click the “Request Appointment” link on any page it is shown and provide the requested information. We will contact you shortly after it is received to inform you of your insurance benefits and setup a date and time for your first appointment. Arrival procedures for your first appointment
Please arrive 15 minutes early for your first appointment if intake paperwork has not been completed. If paperwork has been completed, please arrive 5 minutes early.
Bring your insurance card and a valid photo ID, even if previously provided when scheduling initial appointment. Please also bring a form of payment to your appointment.
We are located in the Oregon Plaza building on NE 20th and NE Oregon street. There is free street parking as well as a dedicated parking lot on the west side of the building
Once you enter the building, please enter Suite 140 on the first floor. It is the door left of the elevator on the first floor. We share an office with Wildwood Physical Therapy. Please take a seat in our waiting room. We do not have front office staff. Dr. Hope Hampton will greet you at your appointment time or slightly before if you still need to fill out new patient paperwork.
What to expect at your first appointment
You will meet with Dr. Hope Hampton for your initial evaluation.
During the evaluation Hope will ask questions about your medical history, current pain, or injury. She will also answer any questions you may have about therapy and your condition.
Functional movements, range of motion, strength, coordination/balance, neurological screen, vitals and much more may be assessed. You will be asked to identify any part of the screening that hurts or reproduces your symptoms. This helps determine the cause of your pain or symptoms.
Hope will recommend plan of care based on your needs. This will include how often we would like see you and discussion around what methods will be used help you reach your goals. Hope will discuss her recommendations with you and answer any questions you might have about them.
There will likely be some treatments performed the first day to get you started on your road to recovery. However, during the next visit you will get into the full swing of your treatment plan.
You will schedule a series of visits with Hope. She may help you schedule them at the end of your appointment or may she call, text, or email you after your first appointment to schedule. Additionally, you are able to schedule your own follow-up appointments online, using the following link.
Your therapist will write up their findings and treatment plan to submit to your referring physician or the medical professional(s) of your choice.
Payment will be collected at time of service at the end of your appointment. Cash, check, and card accepted.

What to expect at your follow up appointments
This is where the magic happens.
Dr. Hope Hampton will begin orthopedic manual therapy treatment of your condition, if indicated.
You will perform and be provided with a series of corrective therapeutic exercises with detailed explanation of how they will help you and correct form. Dr. Hope Hampton will detail how often per day, the number of sets and reps, and the weight/resistance (if applicable). You will be provided with either or both a hard-copy print out and/or the capacity to view your home exercise program online or on your phone.
You will be provided with education surrounding your injury or medical condition, including but not limited to understanding your injury or medical condition, posture, ergonomics, sleep hygiene, understanding pain, benefits of regular exercise, expectations for recovery, when to seek imaging (if necessary), and referral to other medical providers (if necessary).
Dr. Hope Hampton may ask you to obtain certain equipment to use with corrective exercises. Some will be available for purchase in clinic, such as resistance bands and foam rollers, and some may need to be purchased outside of the clinic. Hope will provide you with information about where to obtain the equipment with reasonable good faith estimate of cost to ensure you are paying a fair price.
Payment for your visit will be collected at time of service at the end of the appointment.
What to wear
Please wear loose fitting clothing where your affected body part can easily be observed by Dr. Hope Hampton. Anticipate that Dr. Hope Hampton will observe the skin and surrounding tissue directly with your permission.
