How to Book Your First Appointment

Request online
Click "Schedule a Visit" on any page. You'll be directed to PromptEMR to enter your information.
Complete your intake forms

Before your first appointment you'll receive an email with a link to complete your intake forms digitally. Please do this in advance — it helps us make the most of your first session.

Await confirmation
You'll receive a confirmation email once your appointment is approved. If you don't hear back within one business day, feel free to follow up.
Note on text and email
Text and email are not HIPAA-compliant forms of communication. By choosing to share personal health information via text or email, you do so at your own discretion. For secure communication, the online booking portal is recommended.
Getting Here
Hampton Physiotherapy is located at 1111 SE Stephens St, Suite B, Portland, OR 97214 — in the heart of Southeast Portland.
Parking — Free dedicated lot parking is available directly in front of the clinic. Street parking on SE 11th Ave and SE Stephens St is available for overflow.
Finding us — We are located at 1111 SE Stephens St in Southeast Portland. Look for our signage at the entrance. No need to check-in—I’ll be with you at your scheduled appointment time.
Public transit — The clinic is accessible via TriMet. Plan your trip at trimet.org.
What to Bring & What to Wear
Wear comfortable, loose-fitting clothing that allows easy access to the area being treated. For lower body conditions, shorts are ideal. For upper body or back conditions, a tank top or loose shirt works well.
Bring your insurance card and a valid photo ID to your first appointment.
Bring any relevant imaging — X-rays, MRI reports, or other diagnostic results if you have them. These are helpful but not required.
Bring a list of your current medications if applicable.
Musicians — if your injury is instrument-related, feel free to bring your instrument. Observing you play is one of the most valuable assessment tools I have.
What to Expect at Your First Appointment
Your first appointment is a full hour — just you and me. We'll spend time talking through your history, your symptoms, and your goals before moving into a thorough physical assessment. By the end of your first session, you'll have a clear picture of what's contributing to your symptoms and a plan for how we're going to address it.
You don't always need a referral to be seen at Hampton Physiotherapy — Oregon is a direct access state, which means you can come straight to physical therapy without seeing a doctor first. Learn more on the Direct Access page.
Cancellation Policy
I ask for at least 24 hours notice if you need to cancel or reschedule your appointment. This allows me to offer that time to another patient who may be waiting.
Late cancellation & no-show fee
Cancellations made with less than 24 hours notice are subject to a $100 fee. Missed appointments without notice are subject to a $160 fee. These fees are not billable to insurance and are the patient's responsibility. I understand that life happens — please reach out as soon as possible if something comes up and I will do my best to work with you.
